When meeting with an attorney to discuss how to obtain a guardianship and/or conservatorship of an adult, please provide the following information:
As the “Petitioner” we will need:
- Your name, address and phone number
- Your relationship to the “proposed ward”
- Any information regarding a second petitioner (your spouse, sibling, for example)
- Facts as to why you feel the proposed ward needs a guardianship and/or conservatorship, including information regarding the urgency of the same
Regarding the adult or “Proposed Ward” requiring a guardianship or conservatorship, we will need:
- His or her name, address, phone number, date of birth and current location
- His or her attending physician’s name and phone number
- The nature of the proposed ward’s assets and income, if a conservatorship is being sought
- Information concerning the medical condition and history of the proposed ward
General information regarding other members of the proposed ward’s family:
- Spouse, children, or siblings of the proposed ward
- Their addresses and phone numbers
At the initial meeting, the attorney will discuss the process of obtaining a guardianship and/or conservatorship and answer any questions you may have regarding the process.
