When meeting with an attorney to discuss how to obtain a guardianship and/or conservatorship of an adult, please provide the following information:

Our firm will represent you as   the “Petitioner,” therefore please provide the following information:

1.Your name, address, phone number and email address

2.Your relationship to the “proposed ward”

3.Any information regarding a second petitioner (your spouse or sibling, for example)

4.Facts as to why you feel the proposed ward needs a guardianship and/or conservatorship, including information regarding the urgency of the same

Regarding the adult or “Proposed Ward” requiring a guardianship or conservatorship, we will need:

1.His or her name, address, phone number, date of birth and current location

2.His or her attending physician’s name and phone number

3.The nature of the proposed ward’s assets and income, if a conservatorship is being sought

4.Information concerning the medical condition and history of the proposed ward

General information regarding other members of the proposed ward’s family:

1.Spouse, children, or siblings of the proposed ward

2.Their addresses and phone numbers

At the initial meeting, the attorney will discuss the process of obtaining a guardianship and/or conservatorship and answer any questions you may have regarding the process.
